BALANCE
Care Strategies’ Balance Program is designed to raise participant’s fall prevention, knowledge and awareness. Our customized program is designed to improve strength, mobility, stability and balance for enhanced overall physical health and better functioning in daily activities. Participation may also result in increased self-confidence, better mental health and reduced stress.
BRAIN HEALTH
Cognitive health, which is the ability to clearly think, learn and remember, is an important component of brain health. Other components include motor function, emotional function and sensory function. Care Strategies offers programs that engage participants in mentally stimulating activities that help to increase cognitive health while having fun.
CHRONIC CONDITIONS
Care Strategies provides Chronic Disease Management for anyone requiring a more individualized approach to restoring and maintain their health.
Working one-on-one ensures that the most appropriate programs are recommended and modified for each individual’s needs. We guide each individual through disease management interventions that emphasize activity and wellness that aids in improved independence for a better quality of life. Our programs are ideal for those with a health history that includes CHF, COPD, Diabetes and any other chronic conditions.
EMOTIONAL WELL BEING
Emotional well being is a positive sense of self that allows one to meet the everyday life expectations and function in our societal roles. An individual with a healthy emotional well-being has self-control over one’s thoughts, feelings and behaviors. Good mental health allows one to adapt to changes in life and bounce back from stressors in relationships, work, etc.
If one’s emotional well-being is low, it can lead to challenges in coping with day to day life stressors, feeling “the blues,” sadness, nervousness and often depression. Signs and symptoms of depression can include, but are not limited to, the following: feelings of guilt, hopelessness, irritability, sadness, changes in appetite, sleep changes, loss of interest in day to day activities, crying episodes, difficulty concentrating, and thoughts of self-harm or suicide. If you are experiencing signs and symptoms speak to your physician immediately. If you are having thoughts of self-harm or suicide call 911 or go to your nearest emergency room.
At Care Strategies, we believe healthy, emotional well-being is an essential key to staying healthy and safe. Our goal is to promote active lifestyles, create opportunities for meaningful engagement with others, and build our community relationships.
NUTRITION AND WEIGHT MANAGEMENT
Care Strategies offers a nutrition program that explains the relationship between nutrition and chronic disease, as well as how to improve your diet and overall health. Care Strategies Assisted Living Program can also coordinate a registered dietitian for consultation and provide educational sessions. Meal planning and preparation is also offered for those who qualify in the program.
